Fujishiro Mao is a second-year high school student who loves sweets and wants to be a Patisserie Chef. One day, she heard from her childhood friends Sakuma twins that their brother Itsuki (her first love crush) is returning to Japan from France. Itsuki whom is now a Patisserie Chef plans to open a cafe and Mao later finds out that Itsuki has a girlfriend whilst in France. Extremely heartbroken, Mao was going to throw away her years of Sweets tasting notes that were intended to be given to Itsuki but was stopped by her classmate Keita whom she lends the notes to. Keita comforted and encouraged Mao to continue to pursue her dream and that she should give the notes to Itsuki which later that night she did. The next morning she ran into Itsuki's friend Kyouhei who saw her notes and suggest she should start working at Cafe Cuillere and if possible bring in a few classmates due to lack of staffs. Slightly discomfort with the situation Mao asked Keita to join her on the part-time. Turns out the Sakuma twins and Ryousuke will also be working there too!

You are summoned as a liberator in a world called Femto. A liberator is someone who can perform the ritual of freeing someone's mind. Your goal is to escape the world of Femto and return to your own world, and only way to do so is by freeing people's minds. The game consists of two main gameplay elements. The adventure part which is where the story unfolds and the typical visual novel dialogues are gone through, and a puzzle part which consists of two phases, a phase for moving puzzle pieces, and a phase for moving to a location. The puzzle picture consists of 15 key pieces in total, and you need to put them all in their right place to be able to escape. Each phase gives you limited number of turns and/or points you can spend until it is finished, after which another phase will start. By determining location, you will move onto corresponding adventure part. In each location during adventure element, various events will occur and you will be able to talk to characters there. After the adventure part is over, you will return to the puzzle screen and continue with the next moving phase of puzzle pieces.

Medical 91 is a Japanese-style visual novel. The majority of the game is spent reading and listening to the story unfold, though Yuna will often be given choices that will cause the story to branch in varying ways. At many points, it is possible to move freely throughout the hospital to speak with the other characters on numerous subjects, with the story only proceeding once the proper conversations have taken place. There are also certain segments where the player must choose locations to escape pursuit, as well as avoid capture in a timing-based minigame.
